Consent for Participation in the Coreway Study Phase

I consent to the processing of my personal data in connection with the use of the Coreway health app. This consent includes the processing of my data as part of the registration, use of the app, and participation in the study.

1. Study Duration and Data Collection Scope

Study Duration: Three months
Daily Participation: I will be asked to fill out a short questionnaire in the app every day.
One-time Medical History Questionnaire: Upon first using the app, I will be asked to complete a one-time medical history questionnaire containing questions about my disease progression, including:

  • Date of diagnosis
  • Average number of disease flare-ups per year
  • Severity of flare-ups
  • Other relevant factors

Continued Use of the App After the Study:
After the three-month study period ends, my data will not be automatically deleted, and I can continue using the app. I may choose to keep logging daily entries and track my health and lifestyle factors over a longer period.

Optional Data Deletion:
If I no longer wish to use my data, I can delete my account at any time via the app. This will permanently remove all personal data.

2. Scope of Data Processing

My consent includes the processing of the following data:

During Registration:

  • Username
  • Email
  • Password
  • IBD diagnosis
  • Gender
  • Year of birth
  • Average number of disease flare-ups per year
  • Symptoms & known flare triggers
  • Medication intake
  • Other medical conditions

During App Usage:

  • One-time medical history questionnaire with detailed disease history
  • Optional collection of heart rate variability (HRV)
    • Manual entry or
    • Linking a wearable device via the Terra API
  • Lifestyle factors (usable without HRV data):
    • Sleep quality
    • Stress level
    • Nutrition
    • Physical activity
    • Additional influencing factors such as alcohol and caffeine consumption

Processing by Third-Party Providers:

  • Data storage in the backend via Firebase (Google Cloud) (DPA with Google in place)
  • Management of inquiries and feedback via HubSpot (DPA with HubSpot in place)

I understand that my consent explicitly includes the processing of my health data.

3. Purpose of Data Processing

My data will be processed for the following purposes:

✔ Research and Early Detection:
The data will be used for scientific research on the early detection of disease flare-ups in chronic inflammatory bowel diseases. The analysis takes place within the Coreway Study.

✔ Anonymized Research & Development:
My data may be processed in anonymized form by Coreway and a research institute in Denmark for research purposes. A personal identification is not possible.

✔ Personalized Evaluations (Paid Feature):
I have the option to purchase Coreway Insights, a paid service that provides data analysis and personalized recommendations. This requires a separate subscription.

4. Voluntariness & Withdrawal

My consent is voluntary. I can withdraw it at any time without providing a reason. The legality of data processing carried out before withdrawal remains unaffected.

Data Deletion: I can delete my account at any time via the app. This will completely erase all personal data. Already anonymized data cannot be deleted as it no longer contains any personal identifiers.
